This form may only be completed by someone associated with the event on your campus. ALL reports are subject to verification. Any school submitting poor ratings must supply reasons for the response. We will NOT accept report cards made more than ninety (90) days following an event. We will NOT accept report cards from artists or agents. This report is not considered authentic unless it is dated, a telephone number listed, and E-Mail address provided. Other schools may want to contact you to get an update on your experiences. SUBMIT SEPARATE FORMS on EACH artist or program unless it was an event booked as a package through the SAME AGENCY. You do NOT have to rate an artist in every category unless it applies to you.
